for Automatic Payment Plan [36K]
This form allows the utility to bank draft your checking or savings account each month. You will still receive a monthly statement with your account billing information.
Automatic Credit Card Payment Agreement [24K]
This form allows the utility to charge your MasterCard or Visa card each month. You will still receive a monthly statement with your account billing information.

For Payment Form [30K]
This form allows a property owner to give the utility permission to bill a tenant directly for water and wastewater service.
Credit
Check Authorization [28K]
If you are a new customer without a 2 year credit history at our utility, this form gives you an opportunity to have the deposit waived if you receive a satisfactory credit report.
Authorized
Agent Form [28K]
This form allows the utility to communicate directly and/or render your billing statement to your authorized agent. This form is generally used by customers who utilize the services of a property manager.
